Consumerviews invites you to provide your opinions about products and services that you use every day. If you are interested in participating, simply complete the following brief questionnaire.
This questionnaire will provide us with background information and your email address. We will contact you periodically and ask if you are interested in completing a questionnaire. When you complete a questionnaire you will receive a payment or a gift to express our appreciation.
You will probably not complete a questionnaire more than two times per year.

Participants will be invited to answer questions on a wide variety of subjects. For example...
When you are invited to complete a questionnaire for a client, you will be told what the token payment or gift will be for participation. Typically, participants will receive a check for $5.00 or perhaps coupons worth $5 or more on the client's products. In some situations, participants may receive larger payments.

To receive the payment or gift, you will be asked to fill in your name and mailing address on the questionnaire you complete. This is how your gift will be sent to you. While several members of a single household are allowed to participate in the Consumerviews program, only one person per household is allowed to participate in each questionnaire. Therefore, only one gift will be sent to a household address. Participants who attempt to enter duplicate completed questionnaires will be removed from the program and not invited to participate in future questionnaires.

You will receive your gift or check within four weeks.
